  • If more than one list each one
  • Date you want the Sign Up posted on the Lib Website Members Only and Date you want the Sign Up Closed
  • Check off what info you want the responder to supply you
  • Other Info you want in the response and any other info or instructions to Website.
  • Name and Email of person who will track responses. You must track the responses. The website will send the List Keeper an email every time someone signs up on line . Also list the maximum number of responses you want (i.e.: for bus trip # of seats).
  • Names and Emails of any other person you want a notification email sent to every time a sign up response is submitted.
